POSITION TITLE:              RN's - FACT Team

LOCATION:                           3677 Central Avenue

QUALIFICATIONS: This position requires an Associates Degree or high and graduation from an accredited nursing program, a Registered Nurse’s license in the State of Florida and at least one year experience in a psychiatric setting.  This is a Florida licensed professional position work closely with the program psychiatrist, monitor medications and prescriptions, document effectiveness and response to medications or side effects and serve as members of individual treatment teams.

POSITION TITLE:              Licensed Therapist - FACT Team

LOCATION:                           3677 Central Avenue

QUALIFICATIONS: This is a licensed independent clinician that is responsible for providing all aspects of adult behavioral healthcare services.  This position requires clinical skill and experience to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to program clients with severe and persistent mental illness.   Florida Licensed LMHC or LCSW required.

POSITION TITLE:        FACT Team Leader       

LOCATION:                   3677 Central Avenue                                      

QUALIFICATIONS:  Master’s degree in nursing, social work, psychiatric rehabilitation, or psychology, or is a psychiatrist, LCSW, LMHC, or a certified psychiatric nurse in a related field. This is a licensed independent practitioner eligible to provide all aspects of adult behavioral healthcare services without direct clinical supervision.  Responsibilities include the clinical and administrative supervision of the team and also functions as a practicing clinician responsible for administrative oversight of team operations and a variety of skilled treatment, rehabilitation and support intervention services provided off-site in the community.
