Assertive Community Treatment Team (ACT)

MISSION
The mission of the Home 2 Recovery Program is to assist homeless people to obtain and maintain housing and to live independently in the community.

SERVICES
Assertive Community Treatment (ACT) is an evidence-based model of care. The ACT Team provides comprehensive, community-based treatment to homeless individuals with serious mental health and substance abuse issues who have not benefited from traditional approaches to treatment. This multi-disciplinary team of professionals includes psychiatrists, nurses, case managers, peer counselors and employment specialists. Rehabilitative services, treatment and support are individualized, depending on the person’s needs.
Services include:
  • Assertive case management
  • Assistance in obtaining benefits, employment, food, housing (non-financial), legal assistance, medical care, transportation, vocational services
  • Linkage to community based services
    Medication management
  • Mental health and substance abuse treatment

GOALS
The Home 2 Recovery Program is dedicated to providing homeless individuals with substance abuse and serious mental health issues with the support, treatment, and rehabilitation services they need to continue living in the community. Our goals include:
