Florida Assertive Community Treatment Team (FACT)

The FACT team is comprised of multidisciplinary mental health staff that includes a psychiatrist, nurses, therapists, case managers, vocational counselors, substance abuse counselors, a peer counselor, and support staff.

The mission of the FACT teach is to promote, develop, and support high quality assertive community treatment services that improve the lives of people with serious and persistment mental illness by promoting recovery and empowerment through partnering, self-determination and personal choices.

CBH operates three FACT teams serving Sarasota and DeSoto counties, Charlotte and Hendry counties, and Lee County. FACT is based on a service delivery model that provides comprehensive, community-based treatment to persons with severe and persistent mental illness. The program is dedicated to assisting people in maintaining independent community living. FACT is based on an innovative treatment model that began more than 25 years ago in Madison, Wisconsin.

The goals of the program are to help participants spend significantly more time in independent living situations and experience a reduction in symptoms and hospitalizations. The program also assists participants with employment opportunities and the development of positive social relationships.
