FACT Team

Florida Assertive Community Treatment Team

MISSION:
The mission of the Florida Assertive Community Treatment Team (FACT) is to promote, develop, and support high quality assertive community treatment services that improve the lives of people with serious and persistent mental illness by promoting recovery and empowerment through partnering, self-determination and personal choices.

SERVICES:
The FACT Team is dedicated to assisting severe and persistent mentally ill people who do not respond to traditional services such as outpatient or day treatment. Our individualized care includes:

Activities and life skills
Case management
Housing assistance
Individual and group therapy
Linkage to primary care and dental services
Medication management, administration and monitoring
Psychiatric services
Vocational assistance

GOALS
This evidence based model of care is designed to assist people to live as independently as possible in the community. The focus is on rehabilitation and recovery using individualized treatment plans.
Goals include:

Advocating for greater satisfaction and personal choices in all aspects of daily life
Fostering physical and mental well being
Nurturing healthy social relationships
Pursuit of paid employment
Reducing number of hospitalizations
Reducing symptoms
Support of education
