Prior to your initial visit, please fill out the new client forms and release of information and review the list of helpful paperwork to bring with you.

What to Bring to Your Initial Visit

  1. Records that would be helpful to your assessment
  2. Insurance Card
  3. Payment for services
  4. If you do not have insurance, you may be eligible for a sliding fee scale discount. Please provide sufficient proof of household income (proof of income for every person living in the household that receives any type of income).
    1. SS wage report
    2. Tax return
    3. Pay stub
    4. Bank Statement 
    5. Any other documents you feel would be important in regard to household income
