Adult Supervised Living Services

The Audubon Home and Heritage Home are Level 2 Residential facilities, offering a supervised environment 24-hours per day, seven days per week for adults, age 18 and older, who are unable to live independently due to functional limitations precipitated by chronic mental illness.

Both Audubon Home and Heritage Home seek to reintegrate persons with chronic mental illness into the community and ultimately prepare them for independent living. This is achieved through the provision of a furnished room, meals, assessment services, case management services, nursing services, adult day treatment, on site therapeutic services, crisis intervention, psychiatric services and on site milieu therapy.
